You have given the reason that the policy has been started from year 2008 but in actual policy has been started from 2007 with policy no.[protected] . Please look in to the case accordingly as per your GOLD POLICY all the pre-existing disease will be covered from the third year.

We would like to inform you that, your health claim has been denied, On perusal of the claim documents it is found that patient was admitted ENT Hospital on 09/04/09, diagnosed as DNS left with spur ITH and has undergone septoplasty and partial turbinectomy for the same.

Further, as this is the second year of policy coverage declared by insured in the claim form and inception of the policy is from 26/03/2008 and from the discharge summary of ENT Hospital it is clearly evident that patient is complaining of recurrent cold and sneezing since 1 year. Therefore, this particular ailment is found to be present prior to the inception of policy and going beyond the coverage of present policy.

However, we regret our inability to admit this liability under the present policy conditions.
Hence, claim is being repudiated under policy exclusion clause 1 and condition 2 of above-mentioned policy. We also reserve the right to repudiate the claim under any other ground/s available to us subsequently.

Reliance General Insurance — Hospitalization Claim Denial

I have a Mediclaim policy through Reliance Health Insurance and my policy no. is 1304/[protected].
On 13th April 2010,I had a severe slip Disc problem and was unable to move my lower part of the body and was taken to Central Hospital, Faridabad in an Ambulance. Reliance declined my claim stating that there was no need of hospitalization and it could have been rectified in OPD.

We did ask them to provide us the reason for decline ion writing which they refused and were even rude to my father over the phone. Even there was an investogator who came from the TPA department as per him asked me what is the problem and I explained him everything. While writing the report I could make it out that he was unable to write english and was not educated enough.

As per the Doctor, this was a genuine case and hospitalization was necessary.

I would like to request you to please update me as to where should I contact to get this matter resolved as I would like to take it ahead because the team of Reliance whom I spoke to and my father spoke to were highly unprofessional and I would like to have the reason for denial in writing.
