Hi, My self is Ashutosh Shukla. I was working in Fidelity Gurgaon from dec 2006 to oct 2008. and my provident fund account was in RPFC Gurgaon during this period.Now I am working in CMC limited and I have given the Provident fund/ Pension fund transfer request to my company on 10 th april 2009 and they have submiited the form no 13 to your office on 19th april 2009 but stll the transfer is not happened.CMC limited again intimated you on 21 july 2009 for this request but they have no response yet. below is the details: Name of Employee: Ashutosh Shukla Employees Provident Fund Account No (Gurgaon) : HR/GGN/25531/7246 Employees Pension Fund Account Number (Gurgaon): HR/GGN/25531/7246 CMC Provident Fund Account No : AP/5954/126212 CMC Pension Fund Account Number : AP/5954/126212 Please update me on this mail and make the transfer in my new company PF account as soon as possible. RPFC GURGAON — PF withdrawn submitted june 2010.HR/GGN/10693/000301
Dear Sir/Madam,My name is sanjib rath. as per my company record they have submitted my pf form in 21st june 2010.i visited RPFC office month of august,one of executive told me ur form is not being submitted to us.again i cross check with my company AIRTEL...they clearly told me that form has been submitted.i have sent mail to rpfc gurugaon office n mails is bounced...phone number given by airtel is not contactable....its totally harassment to employee after applied for withdrawn epf more than 1 year....
my pf nu is....HR/GGN/10693/000301
plz do the needful ASAP.....
