Star Health Insurance — No Response after Submitting claim

Please find the attached file which clearly mention that it was wrongly
mentioned

The policy number P/131126/01/2012/000568 and claim number 00115871 has
been rejected due to the reason intoxicating drugs / alcohol. However the
documents which i have submitted, was wrongly mention as (smokeing daily 20
times) however the hospital staff is wrongly mention and I saw the same and
i want and check with my treating doctor and he said it was wrongly
mentioned.
