1]The present status of above certificates,

2] When I can surrender the above and get my investment back

3] I am having one Income Distribution Warrant NO[protected] dt.[protected]inv.id/folio number: [protected]

for Rs. 200 uncashed with me as my bank account details have changed due to my transfer.

4] I have to change my address for correspondance: New address is: VASUDEV N JOSHI, MANAGER(CRM), LIC OF INDIA

DIVISIONAL OFFICE, GANPATHDAS DEVIPATH, SATARA-415001, Maharastra state.

5} MY NEW SB ACCOUNT NO IS : 3235, CORPORATION BANK, LIC DO COMPLEX, SATARA.



Sir, due to my continuous transfers from one place to another and change of address was not communicated to you, to I could not recieve the communications which could have been sent by you.
