Dear Ms.Vaishali Naik,

This refers to your letter dated 29th February, 2012 vide ref no - UTIITSL/CGGF 86/2011-12 for claiming the amount as the policy was foreclosed on 1st April, 2004.

I received this letter on the below mentioned address at my grandfather's house.

My late grandfather., Justice Manmohan Kambli., had gifted me., Mr.Sushant Ashok Paradkar with the Children Gift Growth Fund -86 policy during my childhood. I would like to inform you that I do not have any documents related to this policy, however, I can provide you with the proof of identity & address documents such as pan card & light bill etc.

For claiming this amount I called up your office on[protected] and spoke to Ms.VijayLakshmi and she has asked me to mail you the scanned copy of the letter that will enable her to process the claim. Unfortunately, I have misplaced the envelope that had the folio number written on it, hence, I would request you to retrieve the same from your records and assist me in claiming this amount.
