Employees' State Insurance Corporation [Esic] — no treatment benefits for my parents

Myself anish roy, an employee of bpo convergence asansol, west bengal (Office hq - bhubaneshwar, odisha) appointed as an executive on 25th december 2016.
I am contributing towards esi from 1st may 2017 till date. My insurance no. Is [protected]. My total contribution till july 2017 is of 92 days.
On 14th september 2017 i visited esi hospital asansol branch for a health checkup of my mother but there she was denied treatment because as per their instruction my dependants can not get treatment benefit as they are not eligible for it.
When i visted esi office asansol branch, i was said for treatment of my parents there are 2 conditions to be fulfilled as follows
1) my contribution esi contribution should be more than 78 days
2) my number of working days with my company should be 156 days or more.
In any case i have fulfilled both the criteria then why my parents are still denied even basic treatment?

Also please clear me what is the date of appointment coloumn on esi certificate — is it my appointment in my company date or my date of 1st esi contribution
