Dear Madam/Sir, I, have purchased a Euroclean vacuum cleaner (Model - XFORCE, Serial No.[protected]) on 10/11/2013 through your authorised agent at Siliguri(West Bengal) vide order No. 013235, but I have not received its original voucher/receipt till date. I have to submit my gift request form to Countrywide Network, Pvt Ltd, New Delhi for which a copy of this voucher is required to be enclosed with the application form. It is requested that the concerned authorities may please be directed to dispatch my original voucher/receipt at the earliest so that I can submit gift request form in due date.
