Dear Team. I Nakka Kiran would like to inform you that My mother Mrs Nakka Adi Laxmi taken one policy from EXID life insurance vide police no[protected] She died on 14/04/2012 and i made a death claim on 12/06/2012. But they did not settle the claim. they are asking Below requirements and i give answer so many times but they are not give response. Please do needful Pleas find the below 1. Family Doctor's Certificate -- My mother don`t have any illness that`s why we don`t have any family doctor 2. Treatment records of Surgery Done -- My mother Mrs Nakka Adilaxmi did not gone any Surgery 3. Original Voter ID card of Life Assured -- I Dont have My mother`s Voter ID card 4.
