This is to inform you that my father Mr. Hemanta Kumar Chattopadhyay applied for cashless facility as we were not prepared for DSA. ( ANGIOGRAM) which is not an investigation, it's a mandatory requirement for him as per MRI report.
He was supposed to get admitted at FORTIS HOSPITAL, BANNERGHATTA by today evening. But unfortunately the whole process is messed up right now.
We are getting lots of queries from your side which includes -
1. INDOOR CASE PAPER ( ICP), which can be sent only if the patient is admitted.
2. investigation DSA.( which I have already informed that it's not a investigation, it's a mandatory requirement as per MRI report.

The harrasment me and my father going through since morning because of you guys, it's really difficult to handle an unprepared emergency all alone as my father can't walk.

I have attached MRI REPORT & card details for your reference.
