Family Health Plan [Fhpl] — Reimbursement for medical expenses on PRIORITY basis

Dear sir,
My name is Bharat Laxmanbhai daraji.
My claim id : 3144621
It is to inform you that my father has undergone medical treatement in Shree Krishana Hospital karamsad. He was admitted there from[protected] to[protected]. Hospital has filed online cashless claim but it was rejected due to erroneous reporting by hospital in claim report. During Treatment DR.SHIVANI PATEL has erroneous mentioned that the patient is having alcoholic liver disease. Due to that my cashless claim was rejected by TPA.
Later on doctors have also clarified that patient is not having any alcoholic liver disease. Certificate provided in this regard is also provided in my medical bill reimbursement claim. It is to inform you that the patient has never consumed alcohol in his life.
Dr prepared revise Fresh actual report that my father have liver parenchymal disease under evaluation ( not due to alcohol)
I have submitted the same report to TPA for reimbursement on[protected]
I have filed reimbursement claim and the document file is already delivered your office since 2 month ago.
After passing of two months there was no favourable outcome provided by insurance company. They are only saying that the claim is pending with TPA for further processing and also not confirming the expected timeline required for completing the process of disbursing my case.
It's my humble request to you to look into the matter and do needful on the urgent basis as it has already been delayed by more than 60days.
