HDFC Bank — Non issue of Policy

I Sukumar Karmakar issued one Cheque no 363985 dated 26/12/2014 drawn on SBI Dhaban Branch ( Branch Code No 09716) in favour of HDFC Life for an amount of Rs 20000/- which was cleared from my bank. An agent named Souvik Bannerjee ( Mobile No [protected], [protected])took the cheque from me but till date I have not received the policy nor any communications from your end. Please enquire the matter & let me know the status at the earliest.
