ICICI Prudential Life Insurance — regarding my term plan

It is informed to you, i have purchase online your company term plan with application no. Ob12862762. (On dated 25.01.20). I have submitted all regarding documents and completed my medical as per your schedule and your link hospital (Murkey nursing hom chota bazar, junah rto, datala road, chandrapur) on dated 28.01.20 but unfortunately my application is declined. Your company representative not informed by mail, call as well as. Details on online track application portal. After my call on toll free no. Is informed to me application is declined due to in 16 no. Form. My annul income is 4.9 lacks and as per company norms you are eligible after 5 lack. No call or mail received from your end. It is informed to above matter, my increment on effective from april 2019 and form 16 is last financial year. So my current net salary after deduction is 42800 per month in this financial year (I. E.[protected]
In the view of above matter, i informed to your toll free no. And talk with your senior person, she is suggest to me online again apply and submit the bank statement of last 6 months and salary slip of last 3 months. So i have applied with application no. Ob[protected]On dated 08/02/20) again and online portal show all documents and medical report are accepted but i will check next time and again found my policy is declined. I will try your toll free no. And after 1/2 hours they will respond and she is informed to me, my policy declined due to medical reasons. This time again not any call or mail to me from your company.
So, it is requested to you, please share declined reasons of both application/policies and share my medical reports on my registered my email id.

With due respect that, mam i have firstly informed to you, my policies no. seen in your emails are different at many different mails.My requirement is reasons or your underwriting Norms the aforesaid my applications cannot be issued. and my both applications are declined without any intimation..
Further, it is informed to you no medical reports are attached here in your mail. So, Kindly request to you please check once.
In the view of above matter, You know well, lockdown period is extended upto 3 May 2020 as per the rule of Govt. of India. So, it is kindly requested to you please share E-mail id of GRO( Grievance Redressal officer) Sir or any active online portal/site. So i informed or contact to GRO Sir.
So, again kindly requested to you share, reasons or your underwriting Norms the aforesaid my applications cannot be issued & My medical test reports.

Respected Sir/Madam,                                     With due respect that, As per trailing mail firstly i am very confused my policy no. is 66577702 or [protected] , my policies no. seen in your emails are different at many different mails.So firstly used correct policy no. are used for any communication.                                    Further, As per the your 15.04.20 e-mail, No reasons or your underwriting Norms the aforesaid application cannot be issued of both applications No. ( OB 12862762 & OB[protected] and No My medical Reports are attached in the mail or send in my registered E-mail id . So, Kindly request to you please check once.                                    Yesterday i.e. (17.04.20), i have attend a telephonic call from your appointment department of your company from Kajal sainani (([protected]) for talking to this matter briefly with manager at 4 PM on dated 18.04.20  (Appointment fix or i agreed for this communication) but i have no call received from your end..                                     In the view of above matter, You know well, lock down period is extended upto 3 May 2020 as per the rules of Govt. of India. As per the trailing mail, it is informed to you, As per the regulation/Amendment of MCI (Medical council of India) -Proffessional conduct, Etiquette and Ethics Regulation 2002, Notification date 11 March 2002 clause no. 1.3.2
If any request is made for medical records either by the patients / authorised attendant or
legal authorities involved, the same may be duly acknowledged and documents shall be issued
within the period of 72 hours. (Approx 3 days)
But i have request for share, reasons or your underwriting Norms the aforesaid my applications (OB 12862762 &OB 12999939 cannot be issued & My medical test reports.on registered mail id. But no /reports/ response found at your end approx last 3 months approx.
So, i requested to the senior authority of ICICI Pru life, please look matter personally and reply or respond to my calls or mails.

Respected Sir,

It is already informed to you in my previous mail my first application no. OB 12862762 is declined due to financial issue. The form 16 of last financial year[protected] are attached.Because your representative not inform about annum income norms. My increment is effective from 1 April 2020. After your team discussion she /he is advised to me again apply application No. OB 12999939 with last 6 month bank statement or 3 months salary slip.So, i have again apply with all documents and last 6 months bank statement and 3 month salary slip and also share with your whatsup group.
In the above matter, My Gross/Net salary is 42800 per month in this financial year[protected] .The proof of the my annum salary are same attached with policy proposal form. then why my application are declined?
So, Kindly please requested to you Can you reply me .
