Respected Sir, I am an holder of Connection No. 208, Distributor is M/s BHAGWANPUR INDANE GRAMIN VITRA The Problem is that My name in your record is RAJESH KUMAR CHAUDHARI. But my name in all documents (i.e. Adhaar Card, PAN Card and Qualification Certificates) is as Rajesh KUMAR CHOUDHARY. But, my name in the Subscription Voucher is as RAJESH KUMAR CHOUDHARY also my full signature is seen clearly When reported verbally they just kept delaying saying come on such particular day and extending giving some or the reason for delaying Thanking you Yours sincerely Rajesh KUMAR CHOUDHARY Conn. My mother is a holder of Connection No.CX17798294, Distributor is M/s Veera
Anjeneya/Nanganallur, Chennai.

We got this connection in 1999, the Problem is that her name in your record is S.JAYA. . But her name in all documents (i.e., in Voter Identity Card, Ration Card, Adhaar Card, PAN Card and Bank Pass Book) is as S.JAYALAKSHMI.

Now, we changed our residence from Nanganallur to Medavakkam recently and got TTV from our old distributor. When we contacted the Distributor of Medavakkam, they were not accepted our TTV, they are asking for proof for the
name S.JAYA then only they will accept our TTV. In the mean time, the subsidy amount has not been credited to her Bank account till date. Hence I request you to kindly change my mother’s name in your record as S.JAYALAKSHMI urgently and also intimate the same to us.
