Scope complex, core-, Institutional area, lopdhi road, new delhi-110003 Subject: complaint against madhu gas, bhagalpur, bihar regarding serious negligence of duty viz., irregular supply, non-supply of gas cylinder through mobile booking and demand of extra money payment on the part of supplier and non-compliance of timely exchange of leaking gas cylinder despite repeated reminder Sir, This is to bring to your kind attention that i am sasti charan sinha,, vide consumer number:-mt 171 sv no. 971, distributor’s code:[protected] “madhu gas agency, tilkamanjhi, bhagalpur, bihar, who had booked a lpg cylinder on[protected] on mobile, which was not supplied. Then i again booked it on[protected] through mobile. They still did not pay any attention. Then personally i had to go to the agency (I am sick and my age is 72+ years) on 10-11 — 2015 which was supplied to me that very evening. The cylinder was leaking, which came to my notice that very day, but due to office closure time, i complained about the leaking cylinder on 11-11-15, the very next day. No one looked into the matter and the gas constantly leaked within my home for next eleven days. I had to repeatedly phone the gas agency and i also sent a complaint letter to our chief minister honorable nitish kumar, our district magistrate, mr. Adesh titermare and in-charge, consumer forum, bhagalpur on various dates. Note:- a photo copy of the speedpost is attached for your kind retrieval. Still, the agency exchanged the gas cylinder in the evening of[protected], which was not noted in my refill receipt book. A photo of it is attached for your kind notice. My grudge. If any fatal accident would have happened to my family due to this severe negligence, who would have been held responsible for it? An assurance from your company’s side, a condolence meeting and may be, some grant of one or other kind to console the remaining family members after the fatal death, would probably take place. Sir, i request you to kindly look into the matter and take strict action against this gas agency so that consumers like myself may lead a happy and graceful life and remain loyal to your company. I also request you to kindly let me know about the action you are going to take against this faulty agency.
