Indian Overseas Bank — Cannot use debit card

I tried to pay my insurance premium through my iob visa atm cum debit card last week, but I cannot use my debit card, as it displays invalid expiry date/pin number and now my card is blocked. I started using only now after three or four years of the date of issue of the card. even in atms I cannot get money, as it displays request is failed. so kindly activate my card.

24 the Dec 2016, during adding balance to Paytm 2 transition failed but balance is debited..I complained 2 time in bank regarding to this and also do for it but my balance is not refunded till now.
