kotak mahindara bank-Salary account — Need to close my salary account

When i was opening a salary account, i was told that there will be zero maintenance charge after 6 months of salary being credited. Am using more than 2.5 years. But there was maintenance charges debited for past 4 months. And when i tried to transfer a money outside of country, the transaction got rejected from KOTAK side. If i call customer care, i was told they cant help with it as no issues noticed in my account.
