Kotak Mahindra Bank — welcome kit not received

Havnt recvd welcome kit from Kotak Mahindra Bank due to negligence of their employee Neeraj and Hussain. Its been 5 months now.
I had openend my savings account with the Bank in the month of Februry 2018. In spite of regular followup, I didnt recv the welcome kit, which has cheque book, debit card and other details. Probably they have sent it to a wrong address, however, they dont repsond and they are not resolving my problem.

I had shopping coolwinks BT change my mind thn cancel my order my payment is phonepe one week phonepe receive thn after three days my account bt My refund don't receive my account

Multiple times my money got deducted from kotak account for not having minimum monthly balance without any intimation from the bank this month (14th July - 16th July 2018) and i am unaware that kotak actually has a minimum balance mechanism. This is really unfair. Even i didnt get any alert message for we are going to deduct some amount n all..

On 20th June 2018 I tried to do a flipkart payment through my loyal debit card. But unfortunately that transaction got failed.but 1236 rupees got debited from my account. I checked with flipkart they told me to check with bank. I have not got that refund yet
