We have received the following two Id cards against the Oriental Insurance Med claim Policy no. 215391/48/2016/2135 as per following details:

1. Anuradha Aggarwal (MA-Id: [protected]) (Self)
2. Parveen Kumar Aggarwal (MA-ID: [protected]) (Spouse)

In this regard it is to inform you, both the Id cards received have wrong photograph pasted over it.

It is requested to send us the Id cards having the right photographs to avoid any problem later on.
