Dear Sir, I would like to inform you that I have renewed the above Policy for the year[protected] and for the renewal an amount of Rs.9056/- had been deducted from my Bank A/C ( of Bank Of India, Uttarpara Branch, Hooghly, West Bengal ) on last 03/02/2017. But unfortunately till date I did not receive the Policy Document and the money Receipt. Under such circumstances, I would request you to please do the needful at the earliest so that I may receive the Policy documents and the Money Receipt otherwise I shall not be able to receive any medical assistance if needed. It may be noted that I have been harassed in this way last year also. So please expedite the case and send the above documents in my prescribed address.
