I am kurban ali student of nios. I have changed my name from qurban ali to kurban ali i have passed 10th from nios roll no. M[protected] in june 2017.
I request you please change my name in all related nios documents as (Mark sheet/migration/provisional certificate/passing certificate).
I have changed my name in publication in the government gazette (Bharat ka raj pat), voter id card, ration card, pan card & college id card
I have complied all original legal documents for show with you like as newspaper, publication in the government gazette (Bharat ka raj pat), voter id card, ration card, pan card & college id card.
Originals photocopies of all required following documents are attachments with this application:
1: publication in the government gazette (Bharat ka raj pat) 2: two daily newspapers. (English/hindi news peppers’.
2: original affidavit of first class magistrate.
3: attested photocopy of the slc/tc.
4: demand draft payment of prescribed fee of rs. 200.
5: voter id card, ration card, pan card.
