I have deposited as mediclaim premium amount :13259.48/- dt: 2/9/2016 through icici bank for mediclaim.

Application number is ja[protected] phase 7 gold.
I have submitted all documents for claim in various e mail given in their web site but till date not any response received.
Also tried to contact on given various telephone number but not made any contact, either not reachable, switch off or unavailable.

Please help us as we are in financial trouble and facing very problems due to money.

Please do needful and settle claim.
