Punjab National Bank [PNB] — ppf

My ppf account is with borivali west branch mumbai.
Recently i updated the nominee details.
The nominee details updated are incorrect.
In my internet banking view it shows my husband's name twice.
I have already submitted written application twice in the branch but still it is not updated correctly. I also shared the screenshots of the forms which i submitted when i updated the nominee details.
Can someone help at the earliest.
