I have sent a affidavit of signature and address change to karvy and same has been acknowledged for folio[protected] for reliance industries ltd for my physical shares
45 numbers of reliance industries limited shares which i was possessing in physical form since nov 2003, same has been demated two months back i. E in the month of july'2017,
But because of change in address i have not received the bonus shares which was issued by ril in 2009 and any other shares issued on folio[protected] since 2003 as well as the dividend declared sofar.

So you are requested to do the needful and issue the same or guide me to do the needful for the same if any formalities is yet to be completed from my end.
I have been issued a warrant of dividend for 45 shares (Warrant no[protected]env no - 201172) at my resident which i have cashed in my account (Dividend of demated 45 shared is directly credited my account) so totally i got the dividend of rs.990 in my account for year 2016/17 which is for 90 shares
So far i have got only 45 shares of ril in my demat account, remaining 45 shares yet to be issued to me
Please look in to matter and do the needful at earliest
I have been writing to [protected]@karvy.com since last two month but there is no reply

Updated by bps5396 As guided by Karvy consultancy I have sent required document to issue duplicate share certificate for
bonus shares which was issued by ril in 2009 and any other shares issued on folio[protected]
documents have been sent through speed post on 17.11.2018 same has been delivered to Karvy office at Hydrabad.

I have sent all the required documents required by them as hard copy by speed post & PDF file of all these documents by mail to their official mail ID i.e [protected]@karvy.com

I would like to inform and would like to bring in your notice that my 45 shares against (Reliance Industries) - folio number[protected] has been lost on 01/10/18 so i contacted Karvy for reissue of duplicate certificate against lost one
Karvy sent the procedure of issue of duplicate share certificate.
As per procedure i prepared all the documents which includes
1 FIR of lost certificate from nearby police station
2 Indemnity bond on Rs 500 stamp paper as per given instructions,
3 Affidavit on Rs 100 stamp paper
4 Demand draft of Rs 650 in favour of Karvy computorshares PVT LIMITED payable at Hyderabad
Above all the documents sent through Speed post having consignment No EG[protected]IN
Karvy Computershare Pvt Ltd (UNIT:RELIANCE-MDAG)
Karvy Selenium Tower B,
Plot No. 31 & 32
Gachibowli, Financial District
Nanakramguda,
Hyderabad : 500 032, Andhra Pradesh - India
on 17th November 2018
Same has been delivered to above office on 27th November 2018 shows on www.Indiapost.gov.in
but sorry to says that Karvy office of Hydrabad (Reliance Unit) says that it has not been delivered to their office and they asked for PDF copy of all this documents which are also provided them to hydrabad Karvy office.
I am writing again and again to them through mail but they are not responding at all.
In communication they have just ask my folio no which has also been
provided to them
i would like to intervene your office to issue of duplicate share certificate
