Reliance Trends — Used Defective Mismatch Product delivered

I ordered a t-shirt (Ocean Drive Graphic Print Crew-Neck T-shirt) from JioMart.com on the 26th of September, it was processed on the 05th of October and delivered on the 7th of October.

Please find below my concerns:

The t-shirt which was delivered is entirely different.
I received the size in S, while the size ordered was L.
The t-shirt is used and worn out.
The original tag has been removed and a manual tag is pasted on the worn-out place.
Wasn't there any Quality Check done?
