If your policy cover is basic one, all treatments costs having sub limits, You may insist insurer to pay as per terms & conditions of policy or ask insurer to furnish item wise details of claim allowed in writing, insurer will have to provide the same. After providing details if you are not satisfied you may approach insurance ombudsman for your grievance even in case of no response from insurers
