[Resolved]  ICICI Mutual Funds — CONSOLIDATION OF FOLIOS

I wanted category of Complaint Mutual Fund. Since that was not available I ticked stock market.
I wish to consolidate my three mutual funds into one target folio.
Twice this request was rejected. since middle name is either in full or short, nominee is different, bank name registered is different.
Whenever consolidation is rejected, a fresh account statement is sent,but no necessary form are attached. Therefore I made a own form and written down all the needed details and requested to change all as per this detail. I had also attached cancelled bank cheque, pan card photo copy, KYC photo copy verified by CVLKRA. But it did not solve the problem.

Please send all the necessary forms to do needful. My folio nos. are 1484405, 3305239 & 4589728.
