Policy No: SHC[protected] - Claim No. 773472 Insured Name: Shyla B Kutty Sir, My wife Shyla B Kuttty was hospitalization for the period from 6.11.2014 to 11.11.2014 at Indira Gandhi Co-Operative hospital, Kochi and Refferred to PVS Memorial Hospital, Kochi for a Major operation for Hernia, and removal Fibroid etc., She was in PVS Memorial Hospital from the period from 11.11.2014 to 17.11.2014 The documents, all submitted along with claim forms. But the hospital have cleared for 3 days payment only out of 11 days . Kindly register the claim and do the needful.
