Axis Bank — requesting to provide my uan no

Please provide my uan no and pf no as i missed all the details past two years back.
Am ex employee of axis bank pondicherry branch and my emp no :28838. I have requested my hr and pf help desk for my uan many times but i don't get any proper response from them so i please request you to do the needful to get my uan no and my pf no for my further process i'm my life. Atleast if i come to know the status of my pf i would have been very happy. Now i need to know the status of my provident fund.
