Refusal to give Form 16

I worked for CSC India (computer sciences corporation) from 28-Jan-2010 till 01-March-2011.
I sent an email to the company asking for my FORM 16. They refuse to give me Form 16.
Pls advise

Activation of Account

I have opened a salary account in your ICICI bank. I have got Debit/Atm Card also. Its already one and half month over, but my account is still inactive. For that reason only, i have not received my first month salary also. So please activate my account as soon as possible.
