Dear sir/madam

I am working with Peregrine Guarding Pvt Ltd, Vedanta Goa Office as my name is changed in epf before marriage and i have changed my name after marriage for that i have several times submitted joint declaration to employer Office regarding same but they didn't change my name and now they tells me that your EPF issue you have to solve yourself own otherswise they will terminate me from my job.

Dear sir, I want to deactivate my two UAN,[protected] and[protected] company is not giving exit date, company is not cooperating me, since 14 months my PF is deducting and my UAN number is not credited to[protected] number[protected] UAN though I have aadhar card back link. Close soon my UAN no company is going to link

I am R Kavitha, wife of Mr. S. T. R Paaul Raaj (empoyer Id - CH 25535) worked in Ernst & young LLP, DLF, Chennai under Peregrine Guarding Pvt Ltd and his code is PGS[protected] S11.
He passed away on 24.2.20 due to jaundice and he was in service, I have applied composite form of death claim on 5.12.20 at your main office in Peregrine Guarding Pvt Ltd, Nehru nagar, Perungudi, Chennai to Mr. Prabhu HR. When i enquired about the status of the death claim, he told change of name is the problem, so you are going to return it. My husband changed his name as T Paarvendan Raj during the year October 2018 & i have attached the gazzette form for the change of name.But he did'nt change in any of his particulars with new name, instead we have updated the new name for our son & daughter. If you need i can enclose the aadhar, birth certificate of both with both old and new name as a proof. Moreover they told whenever we need any particulars we can apply with new name along with gazzette form and its his mistake for not enrolling his new name in his company. he was sick for past two years and we are unaware of changing the name cautiously and i got his death certificate in his new name T PAARVENDAN RAJ instead of S T R PAAUL RAAJ. It is mandatory to get any new certificate only with the new name as per the govt rules. i have a son to enroll him in college, if my husband PF & pension gets sanctioned, it will be a great relief for our family as we are in need of money. i can't get death certificate in his old name as your employer says, please consider my application and do the needful at the earliest or else give me the fruitful solution that i can do it. Please reply me.
