विषय:-पेंशन ट्रांसफर और नाम सुधार करवाने हेतु।
महोदय,
सविनय नम्र निवेदन है कि मेरे पति श्री नायक सरदार सिंह कुशवाह सिगनल रेजीमेंट से[protected] को रिटायर हुए थे।उनकी मृत्यु[protected] को हो गई है।मेरे पति का रेजिमेंट नंबर 63162574K है और पीपीओ नंबर D-7400 है।मेरे पति के समस्त (Revision of service/Ordinary family pension/discharge book and old PPO number) कागजों पर मेरा नाम शकुंतला देवी है जो की पूरी तरह से सत्य है।
जो नया पीपीओ नंबर (MRAGR[protected] आया है उस पर मेरा नाम गलती से सकुंतला देवी हो गया है।इस कारणवश जिला सैनिक कल्याण कार्यालय पर मैं और मेरे पुत्र कई बार अपील की है कि मेरे नाम में सुधार कर मेरे पति की पेंशन ट्रांसफर करें।
लेकिन वहां पर बाबू लोगों ने मुझसे मना कर दिया है कि पहले आप नए पीपीओ नंबर के अनुसार अपना नाम बदलवाएं। उसी के बाद आगे की कार्यवाही की जाएगी।मैं पिछले १ महीने से तीन से चार बार वहां जा चुकी हूं।मैं बहुत परेशान हो गई हूं।
कृपया करके मेरा नाम सही करवाएं और मेरी पेंशन ट्रांसफर करें। आपकी अति कृपा होगी धन्यवाद।

मैं अपने समस्त कागजात संग्लगन कर रही हूं।

I No 15367166 y Nk P Seenivasan (OSS-cl-1) was forcibly retired on dtd 31 july 2008 due to low med category, I have completed 19 Yrs and 10 months of service at the time of retirement. Pl. send LPC to CDA Allahabad for ACP(HAV) Benefits.

I received corr ppo[protected] suffix no 2101but my corrppo not received my cpcc sbi not received. Pl send another copy my sbi cpcc hyderabad branch as on earliest.Thanks.

My self JC-379985H Sub & Hony Sub Maj (Retd) Chauhan Shiv Pratap Singh Laxmansingh has submitted on line application for 64 GB smart card on 21 Jun 18. Signals Records has not submitted application duly verified to ECHS Dte to generate online application for treatment of my self, spouse and dependents. Temporary slip issued by Signals Records is not considered for treatment. Six months period is much more for submission of verification. On 17 Dec 18 I have spoken to dealing staff, they assured that verification will be completed by end of Dec 18. However still verification is not submitted. You are requested to given mobile number of dealing officer, JCO so that I can speak on the matter.

Sir, I applied for initial echs card for self and dependent on26/08/2021 vide application number [protected] dated 26/08/2021.This application is pending with signals records due to observation on my date of birth.
On December 2021 I rectified observation s and resubmitted for new echs card verification.my date of birth is 05/08/1964
didnt accepted for editing.so that the following is ammendment for further necessary action please.my date of birth is 05/08/1964 and my wife named Nazma begum date of birth is 07/07/1970 and my daughter named Naziya Husain date of birth is 07/10/2000

Request please submitted date of birth my wife named Nazma Begum is 07/07/1970 and verify my application and issue a temporary slip.so I can avail benefit. Thank you, sir
