Voter ID Card lost Complaints & Reviews

dear sir,
i've lost my voter id card bearing no.BJQ5763784. though the original has been lost few months back, i do still have some photocopies of that card.
i am in urgent need of the same to apply for my further Passport, as am supposed to go abroad.
kindly let me know as fast as possible, whether i can get my new card issued as i have the photo copy.as well as, to whom shall i get to inform to?

I have lost my Voter ID Card on 10th of June 2009,

I have lodge a complaint for the lost of the same.

Please suggest me how can i get a duplicate Election Voter ID Card.
