Axis Bank — Did not received Welcome Kit

Dear Sir,

I create salary account in Axis Bank in branch U183.
I received ATM pin and i Connect passwords.
But I did not received welcome kit and ATM card.
What should I do?

My name is Inderjeet Singh and my house address is 620, jaroda gate jesico colony jagadhri . I didnot received my Bank tool kit of Axis Bank , My bill no is EM[protected]IN. Bank has dispatched on 10-april -2010. pls look this matter urgently.
