dear sir, I am a health insurance holder of Bajaj Allianz health insurence service. This is my details. Customer ID: 24177367 Policy No. OG[protected]-00000619 As per your renewal notice dated on 04/08/2011. I have paid the amount through cheque no. 249097 dated 11/08/2011( central bank of india) through agent Joydev Patowari( mob no. [protected]). But i still not received any document like Health Card and Policy from you. I will be kind enough if you send my renewed Policy and Health Card as soon as possible in my address. Thanking you.
