HDFC Bank — not got the policy documents since 3 months

I Had applied for a policy from HDFC life .. policy name was sampurn samridhi...i had given a cheque of rs.12,500 rupees to the agent and form
but did not got the received any response from the agent.. i mailed him several times but now he stop responding and i did not received the policy document yet..hoping for the solution form you .
