dear sir i have taken for pan card form from your authorised service centre there siting people demand for 2 formRs . 10 rs for pan card form but you have mention for your form for genreral information obtain PAN application form from IT PAN service centre (managed by UTTTSL), PAN application form may be issued at free of cost ( so this information aware for that people ) but not agree and he strictaly demand for Rs. 10 form cost. Personnel — FRAUDULENT TRANSACTIONS
I the undersigned here by lodging a complaint that on 8th March 2011 on Rs.50000 worth transactions were made through my Internet banking a/c of ICICI Bank without my knowledge.Rs 50000 worth were paid through my ICICI Bank Net Banking A/c no.[protected] on 8th March 2011.
I have already registered a complaint at ICICI Bank Gandhidham branch where I have an account on 8th March 2011 around 16.50hrs.
I have already lodged a complaint at ICICI Bank customer care on 8th MARCH 2011 around 16.00 hrs Kindly note the Complaint No. SR[protected].
I here by requesting you to consider my complaint seriously and make arrangements for the refund of my money fraudently withdrawn from my A/c.
