My name is Mitesh Mistry Reliance Healthwise Policy number[protected]
I had processed a pre hospitalization clain at Kokilaben hospital
which was Rejected due to evaluation reasons
however Doctor MANDAR DESHPANDE has issued a letter for minor surgery


I had to undergo a minor surgery and the letter was issued by senior
Doctor MANDAR DESHPANDE FROM Kokilaben hospital
I am paying permium and not getting the services promised
