[Resolved]  Reliance Industries Limited — REQUEST FOR CHANGE OF SHAREHOLDERS NAME

Dear Sir / Madam,

I am holding Share Certificate of Reliance Industries Limited vide Regd. Folio No 65443503, Certificate No 16119104 in the name of BENIMADHAB BHATTACHARJEE (My father) and CHINMOYEE BHATTACHARJEE (My mother). Distinctive Numbers from[protected] to[protected], total no. of shares = 25 (twenty five only).

Now that my parents (both father & mother) has expired father on 03.02.2009 & mother on 02.07.2012 and I am the only legal son / hair (I have no brothers or sisters), so I want to convert the same certificates in my name immediately. Also I want to attach these certificates with my DEMAT Account.

I have obtained the Succession Certificate for all..
