[Resolved] State Bank of India — Delay in receiving ATM Card

I had submitted my application for the Visa ATM cum debit card on the 25th of July to Mr Kulkarni in the ATM department of SBI Kutchery Road branch of Allahabad, but I have not received it yet.Today, I had sent my father to him and to our surprise he told him that he had lost the application form duly filled by me and so, I need to fill up the same and the process will start again !! I have an NRE account in this branch for the last 15 years and am quite dissapointed by his behaiviour. He has not only behaved unprofessionally but also wasted my precious time. State Bank of India — delay of my atm card

Respected sir,
i p.raju sending a mail fom mancherial(mandal), adilabad(dist), AP.I opened a new account at mancherial branch and also a applied atmcard forlast five(5)months back.I didn't get atm card till now.
when i applied atm card i clearly mentioned my correspondense address .repeat once again to below my address and account number is
