1. Ref my PPO No S/028410/2011 (Army) dated 28 Jun 2011 (Photocopy attached for ready reference) and mobile conversation between us on 07 Feb 2012.
2. All formalities i.e. alive certificate etc was submitted by me on 12 Nov 2011 at your branch, however a copy of alive certificate is also attached. Till date amount of above mentioned PPO has not been credited.
3. In view of above, I request your honour to credit the above PPO in my SB Account No. [protected] at the earliest.
