HDFC Bank — Cancellation of policy

Sir i have an account in Hdfc bank and bank provide me a credit card in Jan 17. But without my willing of any policy ur bank activated two in no policy in my account I did not apply for such policy. After a long communication they cancel one of them but second still activated at my account. Sir i want to cancel this policy and all the amount related to this policy should be credit to my account back. Detail of the policy is given below
