HDFC Bank — statement for deactivated lost card

I was having hdfc credit card when i was in surat and then i shifted to mumbai year ago. I have lost my hdfc credit card in august 2018 in mumbai, for which i have called hdfc customer care and asked for blocked that card. At that time no dues were remaining on that card, inspite of that they have issued me new card ending with 4723 and i unregistered that card though hdfc netbanking at that time. Since then i never got any notification for same for 1 year.
Now almost after 1year later they have send me statement which demanding rs 576 for renewal of card which i never received from them.
Now they are saying to send card closure form and sign mismatch and declaration form as i forgot which signed i did at time of submitting document.
But even i don't know full 12 digit card no so how can i fill form.
