HDFC Bank — Still not given my relieving letter and provident fund

Dear sir,
My name is vinayak ramchandra mali i have worked with hdfc bank ltd. Khopoli branch in sli dept. My employee ld is - coex 136405 my joining data is 18 dec.2014 and resined date is 11 july 2016 i have completed my notice period 10 aug. 2016. But till the date not yet my pf and releaving letter. And about this situation i have informed you to on the date 10 oct.2016 but not get reply me.
Therefore i request you to please give my releaving letter and providend fund eairly.
