ICICI Bank — Cheque book and atm

I did not recieved my cheque book after one months.and not any information about my atm card. I submit my request last week (13/1/2017).

My atm was lost last month .I didn't do transaction by atm .I need atm card for money transactions. Please send my atm and cheque book my nearest branch that is Bhagheat road branch code is 6963
