ICICI Bank — Gift Card blocked for online shopping

I have received a Gift Card from Tetrapak, Pune in September, 2016.

My Card number is:

I had visited the following link for online registration:
https://www.3dsecure.icicibank.com/ACSWeb/EnrollWeb/ICICIBank/main/ivrmPreOTPAut...⇄

However, not know what birthdate to put in, it blocked my card further saying ' Incorrect details added'

I want to reactivate my gift card as well as check balance in it.

Kindly serve ASAP
