[Resolved] ICICI Bank — Netbanking and debit card gets blocked everytime

I have a salary account with ICICI bank and this is the third month since the account was opened and every month after my salary gets credited and I go to transfer or make payments through net banking, at the first instance itself the system displays due to security reason your account is blocked'' then I get an sms that my debit card is blocked. When I approach the branch, I am informed that I will need to take a new card and for that I need to pay Rs 500/-. This is surely harassment and mental agony to me. I am planning to take legal recourse if the matter is not resolved permanently.
