ICICI Bank — Smab chgs without notification

I am having an salary account in icici bank[protected] rudrapur uttarakhand. Now i am living in bangalore & change my job. My account has been debited some mab chgs without notifying me by sms or mail. When i gone to deposit money only then i see this. My account deduction are

17/11/16 smab chgs 126.68
17/12/16 smab chgs 354.68
30/12/16 c[protected].00
13/01/17 smab chgs 368.20

I request you to plz see this & kindly revert my money only then i can deposit money in this account otherwise i am thinking to close that account.
