ICICI Bank — Wrong charges for demat account

I never request for opening demat account but from 7 years i get charges of rs 500 + every year and for same i have given request to close to head of department and call center but no support.
Last dedusction date was :- 03/04/2017
Transaction remarks :- in[protected] dp chgs till dec-1

Resolution :- please refund for last 7 years charges of demat and close demat account
Worst service ever.
