D3776243 dt 10/05/2012 in the name of deepak sood c-3, 314 b, sfs flats, pankha road janakpuri, new delhi-110058, i have not received the policy till date, though i am paying the premium timely. The first cheque no.808098 dt 09/05/2012 for rs.24, 000/- drawn on axis bank ltd. Was paid to reliance life insurance co. Ltd.
