Sir, the above consumer id is in the name of sri bishwa nath mishra for flat no. O-4, jagat enclave, khajpura, patna.

Sir, the above mention flat was purchased by me on 13/10/2018 and monthly bill is regularly paid by me.

In view of above, it is requested to change the name of above consumer id in the name of manish kumar s/o sri anand mohan prasad singh after duly verification so that i can use this bill as my identity proof.
